And most of the time my answer surprises patients:<\/span><\/p>\n<p><span style=\"font-weight: 400;\">No. Not every meniscus tear requires surgery.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When people hear the phrase \u201cmeniscus tear,\u201d surgery often immediately comes to mind. In reality, this is an old habit. Today we know that treatment decisions should not be made solely by looking at MRI images. The most important factor is the patient\u2019s complaints and the clinical condition of the knee.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What Is the Meniscus and Why Is It Important?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The menisci are two crescent-shaped cartilage structures located inside the knee joint. However, when it is severely damaged or when a significant portion is removed, the risk of knee osteoarthritis may increase in the long term. For this reason, the fundamental principle today is to preserve the meniscus whenever possible.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Are All Meniscus Tears the Same?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Meniscus tears are generally divided into two main groups.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">1. Traumatic Tears<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">These are usually seen in young and active individuals. They can occur during sports with sudden twisting movements, squatting, or after contact injuries. The tear pattern is usually clearer and may sometimes cause catching or locking in the knee.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">2. Degenerative (Age-Related) Tears<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">These are more common after the age of 40. Often there is no clear trauma history. They may coexist with knee osteoarthritis. Therefore, seeing a tear on MRI does not automatically mean surgery.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">When Should Meniscus Surgery Be Considered?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In some situations, surgery may truly be necessary.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Knee Locking<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If the knee cannot fully bend or extend and there is a mechanical blockage inside the joint, a torn fragment may be trapped in the joint.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Young and Active Patients<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Large full-thickness tears, especially in athletes, may be more suitable for surgical repair.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Persistent Pain<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If there is no clear improvement despite appropriate conservative treatment, surgical options may be evaluated.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Associated Ligament Injuries<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">For example, if a meniscus tear occurs together with an anterior cruciate ligament (ACL) rupture, both may be treated during the same surgery.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Types of Meniscus Surgery<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Meniscus surgeries are generally performed arthroscopically (minimally invasive surgery). Small incisions are made in the knee, and a camera and thin surgical instruments are inserted.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Meniscus Repair<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The torn meniscus is stitched together. This is preferred especially in young patients and in suitable tear types. Recovery takes longer, but it is more beneficial for long-term knee health.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Partial Meniscectomy<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Only the damaged portion of the meniscus is removed. Recovery is faster, but long-term monitoring is important because meniscus tissue decreases.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The modern approach prioritizes preserving the meniscus whenever possible.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">We Treat the Patient, Not the MRI<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">MRI images alone do not determine treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Sometimes a tear that appears severe on MRI causes no symptoms.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Sometimes a small tear may cause significant complaints.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Factors Considered in Treatment Decisions<\/span><\/h3>\n<ul>\n<li><span style=\"font-weight: 400;\">Patient age<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Activity level<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Duration of symptoms<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Knee examination findings<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Cartilage condition<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Therefore, the belief that \u201cIf there is a tear on MRI, surgery is necessary immediately\u201d is incorrect.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">If I Don\u2019t Have Surgery, Will My Knee Get Worse?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">This is another common concern.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In degenerative and small tears, avoiding surgery usually does not worsen the knee condition. Some studies even suggest that unnecessary meniscus trimming may increase cartilage wear over time.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, large and unstable tears causing mechanical locking may lead to further damage if left untreated.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, treatment decisions must be individualized.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Conclusion: There Is No Single Correct Answer<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Not every meniscus tear requires surgery.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But not every tear should be ignored either.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The best approach is to evaluate:<\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Imaging findings<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Clinical examination<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Patient expectations<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">If you have a meniscus tear, the healthiest step is to undergo a detailed orthopedic evaluation before making a decision.
